Virginia Giuffre omitted the name of former prime minister who allegedly raped her from memoir because she feared he would kill her, according to her ghostwriter.
Amy Wallace, who spent four years working on the autobiography, said Prince Andrew's accuser was constantly worried about her “physical safety and threats”.
In April, Ms Giuffre — who won an undisclosed payout from the disgraced royal after claiming she was forced to have sex with him aged 17 — died by suicide at her farm in Australia, aged 41.
Nobody's Girl: A Memoir of Surviving Abuse and Fighting for Justice was released posthumously earlier this month.
In the book, Ms Giuffre described being under the control of the late convicted paedophile Jeffrey Epstein and his associates, including allegations against Andrew which he vigorously denies.
“In my years with them, they lent me out to scores of wealthy, powerful people. I was habitually used and humiliated – and in some instances, choked, beaten, and bloodied,” Ms Giuffre wrote. “I believed that I might die a sex slave.”

In the US edition, Ms Giuffre claims she was sexually assaulted by a man whom she had “taken pains to describe in my legal filings only as a ‘well-known Prime Minister.’”
Passages in the British version are nearly identical but refer to the unnamed individual as a “former minister”.
Ms Wallace, an American journalist, declined to identify the ex-PM, but described him as not just a rapist, but a "sadist".
She told Piers Morgan Uncensored: "If you're asking me if I can name that person, the answer is no … she was afraid he would kill her.
"As I've said, every victim of sexual assault weighs cost/benefit… in Virginia's case, physical safety and threats.”
Since Nobody's Girl was published, several people have contacted Ms Wallace claiming to know who the ex-PM might be.
Ms Wallace said: "She (Ms Giuffre) also had threats from people, indirectly or directly, saying: 'Take our names out of your mouth, or else'."
While on Epstein’s Caribbean island, Ms Giuffre described how “a man who raped me more savagely than anyone had before” when she was 18. “He repeatedly choked me until I lost consciousness and took pleasure in seeing me fear for my life,” she said.
“Horrifically, the Prime Minister laughed when he hurt me and got more aroused when I begged him to stop.”
“Afterwards, I tearfully begged Epstein not to send me back to him. I got down on my knees and pleaded with him. I don’t know if Epstein feared the man or if he owed him a favour, but he wouldn’t make any promises, saying coldly of the politician’s brutality, ‘You’ll get that sometimes.’”
Prior to the memoir's release, Andrew relinquished his titles and has reportedly agreed to vacate Royal Lodge in Windsor, Berkshire.
Meanwhile, the Metropolitan Police are investigating claims that Andrew asked his taxpayer-funded protection officer to uncover information about Ms Giuffre.
