The Difference the New ADHD Drug Named Simtriyo Makes and How It Works in the Brain

The U.S. Food and Drug Administration has approved SIMTRIYO (centanafadine) for the treatment of attention-deficit/hyperactivity disorder (ADHD) in adults and pediatric patients ages 6 and older who weigh at least 20 kilograms.

Developed by Otsuka Pharmaceutical, SIMTRIYO is an extended-release medication taken once daily. It introduces a new FDA-approved pharmacological approach to ADHD by inhibiting the reuptake of three neurotransmitters: norepinephrine, dopamine and serotonin. Although its mechanism differs from existing ADHD treatments, the FDA classifies SIMTRIYO as a central nervous system (CNS) stimulant.

The approval was supported by four pivotal Phase 3 clinical trials involving adults and pediatric patients. Otsuka reported that the studies found statistically significant improvements in ADHD symptoms compared with placebo. The company said the medication is expected to become commercially available later in 2026 following scheduling by the U.S. Drug Enforcement Administration.

The approval provides another medication option for people with ADHD while giving clinicians a treatment that works through a mechanism not previously approved for the disorder.

How SIMTRIYO Takes a Different Route Through the Brain

SIMTRIYO works by inhibiting the reuptake of norepinephrine, dopamine and serotonin. Reuptake is the process through which nerve cells remove neurotransmitters from the space between nerve cells. Inhibiting this process can increase the availability of these chemical messengers for signaling.

Centanafadine is the first FDA-approved norepinephrine, dopamine, serotonin reuptake inhibitor, or NDSRI, for ADHD. Its ability to affect all three neurotransmitter systems distinguishes its mechanism from ADHD medications that primarily affect norepinephrine and/or dopamine.

The significance of this mechanism should be kept in perspective. Affecting three neurotransmitter systems does not mean SIMTRIYO will necessarily work better than other ADHD medications for every person. Its importance lies in providing another pharmacological option with a distinct mechanism of action.

The Relevance of Dopamine, Norepinephrine and Serotonin to ADHD

Dopamine and norepinephrine contribute to brain processes involved in attention, motivation, alertness and behavioral regulation. Serotonin is involved in numerous functions throughout the brain, including processes related to mood and emotional regulation.

These systems do not operate independently, and ADHD cannot be reduced to a problem involving one specific chemical. ADHD is a neurodevelopmental disorder characterized by persistent patterns of inattention, hyperactivity, and impulsivity that interfere with functioning. Symptoms can appear predominantly as inattentive behavior, hyperactive-impulsive behavior, or a combination of both.

Identifying ADHD and Addressing It

There is no single test that can diagnose ADHD. Healthcare professionals generally evaluate symptoms, their duration, and their effect on everyday functioning while considering a person's developmental history and other possible explanations. Some conditions, including sleep disorders, anxiety and depression, can produce symptoms that overlap with ADHD.

Treatment can include medication, behavioral interventions and other forms of support. The appropriate approach depends on factors such as a person's age, symptoms, circumstances and response to treatment.

Researchers believe ADHD has multiple contributing factors, including genetic and environmental influences. There is no established method for completely preventing ADHD, making early recognition and appropriate treatment important for managing its effects.

Another Option for People Still Searching for Effective Treatment

Finding an effective ADHD treatment can require an individualized approach. Some people respond well to existing medications, while others may experience inadequate symptom control or side effects that make a particular treatment unsuitable.

SIMTRIYO gives healthcare professionals another option to consider when developing treatment plans. Its approval for both adults and eligible children also means that the medication can be considered across a broad age range.

The Phase 3 program included studies in adults and children and found improvements in ADHD symptoms compared with placebo. Otsuka described the medication's safety profile based on the clinical-trial program, but its FDA-approved labeling contains important warnings that patients and healthcare professionals must consider.

Because SIMTRIYO is a CNS stimulant, its labeling includes a boxed warning concerning abuse, misuse and addiction. The FDA labeling also contains a boxed warning regarding suicidal thoughts and behaviors in pediatric patients. Other warnings include potential increases in blood pressure and heart rate, psychiatric effects, and possible effects on growth in children.

These warnings do not mean that the medication is inappropriate for people with ADHD. Rather, they highlight why treatment decisions should be made with a healthcare professional who can consider an individual's medical history and monitor their response.

The Encouraging Step Forward For ADHD Treatment

The FDA approval is significant because ADHD can affect attention, behavior, school performance, work and relationships throughout a person's life. Having another approved treatment gives clinicians an additional option for patients whose symptoms are not adequately controlled by medications they have already tried.

The importance of SIMTRIYO is therefore not simply that it affects three neurotransmitters. Its approval expands the range of available pharmacological approaches to ADHD and gives clinicians another mechanism to consider when tailoring treatment.

At the same time, the drug should not be viewed as a universal solution. ADHD varies substantially from person to person, and the most appropriate treatment depends on individual symptoms, age, medical history, previous treatment experiences, and response to medication.

SIMTRIYO's approval represents another step in expanding the options available to people living with ADHD. Whether it becomes an effective choice for an individual patient will ultimately depend on how that person responds to the treatment and how its potential benefits compare with its risks.
