A man charged over a fatal crash on Melbourne's West Gate Freeway has been remanded in custody.

Wyndham Vale man Emanuel Kur, 21, did not appear in Melbourne Magistrates Court on Wednesday, instead remaining under police guard at the Royal Melbourne Hospital.

His lawyer Nick Jane told the court Kur had significant physical injuries after last week's crash on the West Gate Freeway.

The 21 year old was allegedly driving a Ford Falcon sedan that collided with a truck on the bridge on the morning of July 5.

One of the passengers in the sedan, a 22-year-old Truganina man died from his injuries in hospital the next day.

Kur was on Wednesday charged with dangerous driving causing death and reckless conduct endangering life.

It is the 21-year-old's first time in custody, Mr Jane said.

Kur did not apply for bail and was instead remanded in custody.

He is due to appear in Melbourne Magistrates Court in December.
