Suicidal mum discharged from hospital killed herself days later

The young mum of an eight-month-old baby tried to get medical help one week before she took her own life.

“Fantastic mum” Chloe McDaid, from Wallasey, was found by her father at her home after she was discharged from hospital .

The 24-year-old had told medical professionals she was suicidal and had sought help with her mental health as far back as 2011, an inquest into her death heard. 

The family added they didn’t want “anyone else” to go through what they had. 

Ms McDaid, who worked as a barmaid and was mum to Melodie, was diagnosed with ADHD and moderate depression and had sought help for anxiety previously. 

She had been described as “the kind of person who would light up a room with her smile.” 

The mum-of-one visited hospital on November 23, 2018, for help with depression and anxiety.

The court heard how Ms McDaid had attended appointments in March, 2019, but subsequently missed later appointments in April. 

On June 3, 2019, Ms McDaid had sought more help and was referred to Bowmere Hospital, Chester.

Despite saying she'd had suicidal thoughts she was discharged on June 11, and then seen by medics again on June 14.

Her family said: “Why was she discharged after only one week?

“Did you think she was alright and no longer suicidal? It takes more than a week to even get over a cold.” 

The court heard Ms McDaid hanged herself shortly after having an argument with her partner, on June 26.

Despite the best efforts of her father and paramedics to conduct CPR, Ms McDaid was placed in an induced coma and died later at Arrowe Park Hospital on July 2, 2019. 

Det Supt Int Phil Atkinson said there was also evidence that Ms McDaid had self-harmed.

Sam Hare, Investigating Manager for Brackendale Ward at Bowmere Hospital, said an investigation had been undertaken into Ms Mcdaid’s death.

She told the court when Ms McDaid was first admitted it was indicated this would be for a ‘short period of time’. 

After a thorough evaluation it was deemed by a multi-disciplinary team that Ms McDaid was ready to be discharged and receive care from other mental health professionals at home.

She said: “There was three assessments carried out while she was on the ward. 

“During these assessments, the main cause of concern raised by Ms McDaid was social and financial issues.” 

She added when Ms McDaid was discharged “she was adamant she had no intention of harming herself.”

Chloe's mum Amanda Boden asked: “Why was she discharged after only one week when she said she was suicidal?  

“Life is precious, so think hard about discharging someone.” 

Concerns were also raised by Ms McDaid’s family about her parents not being consulted during her discharge.

In regard to the family not being present in her discharge meeting, Ms Hare said this was “unusual” and apologised to the family on behalf of the hospital. 

She said: “It’s below standards.” 

Ms Mcdaid’s family also questioned why their daughter was allowed to leave without a working phone. 

Amanda Boden also asked: “Why was our daughter discharged without a phone to contact anybody? 

“She has been admitted to Bowmere because she was suicidal. Our daughter would still be here if she had someone to talk to.” 

Ms Hare said Ms McDaid had been informed about the risks to personal property on the ward and had been told the protocol to replace a damaged phone.

Assistant Coroner Julie Golding said Ms Mcdaid’s cause of death was asphyxiation by self-hanging. 

Giving a narrative finding, she said: "It’s not possible to say if her death was anything other than a desperate cry for help."
