John Holder's verdict
1) Tell the captain to get on with his job: he has no right to object. If the bowler needs a plaster for an injury, he should have a plaster. You're not a doctor, and neither is the captain. Thanks to Stephen Field.
2) It cannot be a six as the ball had been on the ground, but the ball is dead so four runs are scored. On top of that, any completed runs will also count. Thanks to Seb Duggan.
3) The laws here are similar to football: umpires are considered part of the field of play, and a striker can be caught when the ball deflects off them. However, in this instance it would be grossly unfair for the striker to be dismissed because of an umpire's deliberate action, so you should call and signal dead ball immediately, to prevent any further development.
