1) You have to be aware of gamesmanship on both sides. Having rightly cautioned the striker for showing the imaginary card you should then make sure you give him the appropriate protection, to avoid this sort of escalation. Tell him that you're aware the opposition players are targeting him to try and force a second yellow, and at the next opportunity take the opposition captain to one side to issue his team with a public warning that their tactic must stop. With that warning in place, you should then caution the next player who fouls this individual. Thanks to David Millward.
2) Another great question to start the season. "Intent" is no longer in the laws; in this case the defender has simply committed a foul as a result of carelessness – however unintentional it was. So restart with a direct free kick, and dismiss the defender for the denial of an obvious goal scoring opportunity.

3) Under no circumstances accept the shirt on the pitch – whether it's a genuine gesture or not. Thank the captain for his wonderful gesture and suggest that, because you want to photograph the moment, he should keep his shirt on until you can do a formal presentation in your dressing room.
