A woman inpatient in a secure psychiatric hospital was found dying in a bathroom.
The patient was found by staff on the all-female Dee Ward in Clock View Hospital, Walton, in a serious condition.
The woman, who appeared to have made an attempt to take her own life, was rushed to A&E but later died in hospital after the incident, which occurred in recent weeks.

Mersey Care NHS Foundation Trust, which runs Clock View, said investigations were underway and reviews involving other recent inpatient deaths were ongoing to identify any themes.
In April the ECHO reported on the apparent suicide of a male patient on Morris Ward, also in Clock View, who was found dead in his room.
A spokesman for the trust said: "We are aware of the tragic death of one of our service users within our Local Division and our thoughts go out to their friends, family and loved ones at this distressing time.
“We are unable to comment further because the case is now with the coroner and our internal learning review into the circumstances leading up to this incident is still ongoing.
"Once that is completed, our findings will be presented to the coroner.”
According to a Safety Report to the board of governors: "A female service user has sadly died in hospital after being found... in her bathroom, a 72 Hour Review has been completed and shared with Clinical Commissioning Group and Care Quality Commission, a full learning review will commence once the Police have completed their investigation.
"Thematic analysis of actions from recent inpatient deaths has commenced with Risk and Governance Manager and Serious Incident Lead.
"An overarching action plan will be produced which will be shared with Deputy COO/Inpatient Lead and provide evidence for Divisional Lesson Learnt statements for upcoming Coroners Inquests.
"An Inpatient Strategy Group has also been established whose purpose is to ensure that the development and delivery of ‘Local Division Inpatient Services’ takes place in a coordinated and connected way, following the requirements of the NHS Long Term Plan and the Mersey Care NHS Foundation Trust Strategy."
