More than 450 patients died at a hospital in the UK after being given powerful painkillers, a new report has found.
The report says an "institutionalised" practice of administering opioids without medical justifications which led to the deaths.
There are now calls for criminal charges to be brought against the doctor who was at the centre of this scandal.
