Two of Four Cats with Bird Flu in One California Household Survived After Early Antiviral Treatment

Highly pathogenic H5N1 is close to a death sentence in domestic cats. Infected animals typically develop severe respiratory and neurologic disease, and published case reports across multiple countries end the same way.

A case series from Tulare County, California, documents a partial exception. Four cats in a single household, living within about a mile of a dairy farm at the center of California's H5N1 cattle outbreaks, became infected between late October and mid-November 2024. Two died. The two who received early supportive care and the antiviral oseltamivir recovered.

The report, published in the journal One Health after circulating as a preprint, is unusual not because cats caught bird flu but because two of them lived.


An Outbreak Inside a House

The household sat near a dairy farm in Tulare, which the authors describe as the epicenter of California's H5N1 outbreaks in dairy cattle. The cat owner delivered hay to local dairies.

The first cat died suddenly at a local veterinary clinic after developing breathing problems. The second died within days at a different clinic despite fluids and antibiotics. Neither received antiviral treatment before death, and neither was tested for H5N1 at the time.

The clinical picture in one of the survivors illustrates how this presents. Cat 4 arrived febrile at 103.4°F, weighing 5.36 kg, with signs of upper respiratory infection: purulent nasal discharge from both nostrils, watery discharge from both eyes, and increased bronchovesicular sounds bilaterally.

Treatment was supportive and antiviral. The cat received subcutaneous fluids, an injectable anti-inflammatory, and oseltamivir twice daily for ten days, transitioning after 24 hours to an oral anti-inflammatory.

The authors' conclusion is deliberately narrow: with early supportive care and early antiviral treatment, H5N1 is survivable in domestic cats. They frame the results as supporting continued prioritization of neuraminidase inhibitors for H5N1 in mammals and as underscoring the value of early identification.


What Four Cats Can and Cannot Establish

This is a case series of four animals in one household. It is not a trial; there was no comparison group, and no one can say from this data how many untreated cats in identical circumstances would also have recovered.

Cats that reach a veterinarian early enough to be treated are, by definition, cats whose owners noticed something and acted. That selection alone could explain a better outcome without any drug being involved. The two cats that died were also seen by veterinarians, but their illness was recognized too late for antivirals to be attempted.

The strength of the report is that it documents survival at all, in a species where the published record is dominated by fatalities and where treatment has rarely been attempted before death or euthanasia. As the University of Maryland School of Public Health noted in its description of the work, the researchers emphasize early intervention as the decisive factor.

The second finding is arguably more useful for surveillance. Blood samples collected from the recovered cats showed high titers of neutralizing antibodies against H5N1 at least three to four months after recovery, suggesting that cats that survive natural infection may retain meaningful protection against reinfection.

Pet owners should not read any of this as a treatment plan. Oseltamivir use in cats is off-label, dosing in animals is a veterinary decision, and a cat with respiratory or neurologic signs needs a veterinarian, not a human medicine cabinet.


How Indoor Cats Are Getting Exposed

The exposure routes identified in US investigations are not what most owners would guess, and one of them requires no outdoor access at all.

In May 2024, Michigan health officials investigated H5N1 infections in two cats, both described as exclusively indoor and living in different households, both presenting with respiratory and neurologic illness. Both cats died. One owner worked on a dairy farm. The other lived alone, worked on multiple dairy farms transporting unpasteurized milk, and reported being splashed in the face and eyes with it. Both workers declined influenza A testing.

Neither cat left the house. The virus appears to have come home on clothing, boots, or hands.

The other established route is diet. Federal investigators have repeatedly linked feline H5N1 infections to raw milk and raw meat pet food. FDA has issued notices to pet owners after testing linked contaminated raw cat food to the death of a cat, and the authors of a CDC report on cat-associated exposure advised plainly that pet owners should not feed raw animal products to cats.


The Person at the Other End of the Leash

The reason any of this belongs in a health publication rather than a veterinary one appeared in the CDC's Morbidity and Mortality Weekly Report in May 2026.

Los Angeles County investigators reported serological evidence of H5N1 infection in a veterinary professional who was exposed to an infected domestic cat. Between November 2024 and January 2025, the county identified 19 domestic cats from five households that became ill after consuming commercially purchased raw milk, raw meat, or raw pet food. Fourteen died or were euthanized. Nine were tested, and all were positive for H5N1 clade 2.3.4.4b genotype B3.13.

A total of 139 people exposed to those cats were identified and monitored. Twenty-five volunteered for serologic testing. One, an asymptomatic veterinary professional who had direct contact with an infected cat without eye or mouth protection, was seropositive. The authors wrote that the findings provide evidence of zoonotic transmission of H5N1 from domestic cats to humans, and the American Veterinary Medical Association described it as the first documented case of this.

That is a single documented instance identified through antibody testing, not a pattern of sustained spread, and it should not be read as evidence that cats are driving human infections. There is no evidence of sustained person-to-person transmission of H5N1.

What it means is that a sick cat in a dairy-adjacent household is both a public health and a veterinary event. Owners whose work involves livestock should change clothes and shoes before contact with pets, avoid feeding raw milk or raw meat products, and tell a veterinarian about occupational exposure when a cat develops respiratory or neurologic illness. Anyone who develops fever, cough, or eye redness after handling a sick animal should contact a clinician and mention the exposure.
