A man has been taken to hospital after a truck and train collided at a level crossing in the Upper Hunter on Wednesday morning.
Emergency crews were called to the scene near Scone just before 7.15am.
NSW Ambulance paramedics treated the truck driver - a 29-year-old man - at the Turanville Road scene.
He was taken to Scone Hospital with suspected head and chest injuries before being air-lifted to John Hunter Hospital.
"When we arrived on scene and saw the wreckage, there was obviously some serious concerns that anyone involved could be critically injured," NSW Ambulance station officer Mark Bell said.
"The local community members who rushed to his aid should be applauded, it's great to see people looking out for each other in such difficult moments.
"It's so important to maintain vigilance when driving any type of motor vehicle. It only takes a split second for a horrific incident like this to occur."
Hunter Valley Police District officers are investigating.
The rail line in the area is closed and is expected to remain so while initial inquiries are completed.
