- Shaun Burton, a 60-year-old train driver, died in a train crash in Bedfordshire on 19 June.
- His East Midlands Railway (EMR) train collided with the back of a stationary EMR train near Elstow, Bedford, injuring 162 people, with 102 requiring hospital treatment.
- An interim report by the Rail Accident Investigation Branch (RAIB) indicated Mr Burton's train proceeded past a red signal before the collision.
- An inquest heard Mr Burton's provisional cause of death was "traumatic injuries to the brainstem and chest".
- The inquest has been adjourned pending the conclusion of investigations by the RAIB and British Transport Police .
