McCann, 34, had been released in error from jail in February when probation officers saw him repeatedly without realising the mistake.

His final probation meeting was three days before he abducted his first victim, when he was given a slap on the wrist for breaching his licence conditions.

He went on to commit a string of sex offences against 10 more victims aged between 11 and 71 in London and across the north of England.

Justice Edis called for an independent inquiry as he handed McCann 33 life sentences with a minimum of 30 years.

He told the Old Bailey: “There should be a systematic and independent investigation reported in public into how the system failed.”

The judge described McCann, who refused to appear in court, as a “classic psychopath”.

He said: “You are very dangerous to people who are weaker than you are. Among other things, you are a coward and violent bully and a paedophile.

“This was a campaign of rape, violence and abduction of a kind which I have never seen or heard of before.”

Paying tribute to his victims, the judge said: “Each of these stories is tragic. I hope each will find a way to live through their ordeal and future happiness.”

McCann has spent most of his life in jail, having received his first sentence aged 15.

He had a “history of violence and threats towards partners” but no convictions for sexual offences.

In an impact statement, one of McCann’s victims said: “My aspirations, both small and big, and my vision of a positive future, have been violently taken from me.

“To replace a life of thriving with one of surviving is deeply demoralising and difficult.”

The Probation Service has issued an “unreserved” apology.

A probation officer was demoted after a review found McCann, of Middlesex, should have gone before the Parole Board instead of being released.
