Police are investigating a head-on crash on Queensland's Sunshine Coast that left a three-year-old boy with critical injuries and five others hurt.

Police say two cars collided on the Sunshine Motorway near Coolum Beach just after 2pm on Monday.

The three-year-old boy has been flown to the Queensland Children's Hospital in Brisbane for treatment.

A 36-year-old man and a three-year-old girl who were also travelling in the car with the boy have been taken to Sunshine Coast University Hospital in a stable condition.

A 38-year-old woman driving the other car and two children aged 12 and four have also been taken to hospital for treatment.

The Forensic Crash Unit is appealing for anyone who may have witnessed the crash or has dash cam footage to contact police.
