Chelsea's tactics proved too much for City as they forced the Premier League champions to play narrow before looking for a threat on the counter-attack.
It meant the Chelsea defence was barely troubled throughout the contest as the Blues lifted their second Champions League title.
Manager Thomas Tuchel has also avenged his own defeat in last season's final when his PSG side were comfortably beaten by Bayern Munich.
The German manager dramatically improved the Blues after taking over from Frank Lampard in January and this tournament win will go down in Stamford Bridge history.
