Public Health Experts Say the Record Cyclospora Outbreak Exposed a Communications Failure, Not an Investigative One

The largest documented Cyclospora outbreak in United States history was traced to a single farm in central Mexico within weeks, using interviews and shipping records, because the parasite defeats the genetic fingerprinting that normally solves foodborne outbreaks.

That investigation worked. What failed, according to an opinion piece published by CIDRAP, was the explanation.

The argument comes from Jess Steier, DrPH, a public health scientist and founder of the science communication organization Unbiased Science. CIDRAP notes that opinions in its op-ed pieces are the authors' own and do not necessarily represent the center's position. This article reports her case; it is analysis, not a news finding.


The Central Claim: Different Numbers Measuring Different Things

Steier's core objection concerns arithmetic that has appeared across much of the coverage, including some of our own.

Cyclosporiasis has been reported in 41 states. The outbreak linked to iceberg lettuce is in nine. Those are different measurements. CDC and FDA count 1,947 confirmed cases in the nine-state outbreak that epidemiologic and trace-back evidence ties to lettuce from a single supplier. Separately, CDC reports more than 4,000 confirmed domestically acquired cyclosporiasis cases nationally since May. Thousands more patient samples await confirmation, and several hundred additional cases involve travelers who became ill abroad. Some states publish probable and confirmed cases together.

Stack all of it together, she writes, and "you get a single national outbreak that doesn't exist."

The consequence she identifies is not merely public anxiety. Folding separate signals into one running total hides the possibility that several outbreaks are happening at once. Several states, including California and New Jersey, have said their counts are running in line with typical seasonal patterns. North Carolina attributes its increase to parsley and cilantro, neither of which has emerged as a national signal. FDA is separately investigating a cluster of at least 72 people with no source identified.

Cyclosporiasis appears every summer in a well-established seasonal pattern, frequently tied to fresh produce. A cumulative tally, she argues, is a surveillance instrument that cannot tell an individual whether they are at risk this week, which is the question people are actually asking.


Why the Investigation Was Genuinely Hard

The piece is careful to credit the epidemiology, and the technical constraints are worth understanding.

Cyclospora will not grow in culture. For most foodborne bacteria, a laboratory grows the organism, sequences it, and uploads a genetic fingerprint to PulseNet, the national network that flags when cases in different states match. That option does not exist here. CDC has been working from partial genotyping because the parasite's genome defeats sequencing methods designed for bacteria.

Contamination on produce is also sparse and unevenly distributed, sitting near the limit of what a test can detect. That is how contaminated lettuce can test clean.

Investigators were left with interviews and shipping records. Michigan worked through ingredient-level food histories from 190 people who had eaten at Taco Bell before falling ill, and 90 percent had eaten iceberg lettuce. FDA trace-back followed the paperwork to one supplier and then to a single independent farm in central Mexico, which Taylor Farms has said accounts for less than 1 percent of the US iceberg supply. The recall came July 17.

An early demographic clue was available and widely missed. Cases skewed heavily toward adults, with a median age of 44 across a range from 1 to 89. The relative absence of young children pointed toward a vegetable rather than fruit, since children eat far more fruit than greens.


The Timing Problems and the Retracted Test

Two specific sequencing failures anchor the critique.

CDC's Health Alert Network advisory arrived July 14, roughly 10 weeks after the agency began receiving reports of a national rise in cases. When CDC published an epidemic curve on July 24, plotting the 1,947 outbreak cases by illness onset rather than report date, it showed two distinct peaks, one cresting around June 25 and another around July 10, with a deep valley between. Working back through the parasite's roughly week-long incubation period places exposures near mid-June and again at the start of July, which Steier argues is more consistent with separate exposure windows than one prolonged stretch. The first peak had risen and fallen before the advisory was issued.

Then came the test. FDA published notice of a Cyclospora-positive lettuce sample on Saturday, July 18, retracted it the next day, and did not explain the sequence until the afternoon of July 20. The sample had come from routine border screening rather than recalled product, and the case implicating the supplier never rested on it. By then, the phrase false-positive had run the news cycle, and many people came away believing the company had been cleared.

The cost of getting a warning wrong, she notes, is paid twice: once by whoever was wrongly named, and again by every future warning the public quietly stops believing.


The Two Audiences Problem

The most practically useful part of the argument is about who guidance was written for.

For someone on chemotherapy or living with a transplant, cyclosporiasis is not a rough week. It runs longer, relapses, and can bring complications requiring clinical attention. Telling that person to avoid lettuce while the source was unknown was sound advice.

For most of the country,y it was not, and Craig Hedberg, PhD, at the University of Minnesota has argued through the month that broad avoidance guidance without a known source does more harm than good. Both messages were correct. Only one went out.

For readers now, the specific product has been recalled and consumers should not eat recalled iceberg lettuce from the named supplier. Cyclospora is not spread person to person. Cooking to an internal temperature of 158 degrees Fahrenheit kills it, and washing produce does not reliably remove it. Anyone with sudden, prolonged, or relapsing diarrhea should ask a clinician specifically about Cyclospora testing, because many routine stool panels do not include it.

FDA has said confirmed counts will keep climbing well after the recall, because of the long lag between exposure, testing, and confirmation. Rising numbers in the coming weeks will not mean new exposures are occurring.

The argument being made is that the investigative work was sound and the public explanation was not. The people most affected are immunocompromised readers who needed specific guidance and everyone else who received guidance meant for them. The most reasonable action is to check whether a product is on the recall list and to request Cyclospora testing if symptoms fit. The central uncertainty is whether more than one outbreak has been running at once, which the merged totals make harder to see.
