Paul Gascoigne has been pictured clutching his shoulder in pain shortly before being rushed to hospital.
The former footballer is taking part in Italian reality show L'Isola dei Famosi - Celebrity Island - which is filmed on the island of Cayo Cochinos off the coast of Honduras
He is said to have hurt his shoulder during a challenge and was treated by onsite medics before being whisked away to be seen by doctors.
The Sun reports Gazza was taken to a medical centre for X-rays after injuring himself while playing a ball game which gave the contestants the chance to win a spaghetti dinner.
The task is believed to have involved catching a ball and crawling through a ditch before climbing a cargo net onto a platform to shoot it through a basketball hoop.
Pictures now show Paul grabbing his shoulder and grimacing as he waded out of the sea.
Show host Ilary Blasi is believed to have noticed Paul looked like he'd hurt himself, saying: "What's happened to Paul, he looks in trouble?"

One of the other contestants, actor Gilles Rocca, is said to have gone over to the former sports star and comforted him before he was led away off camera.
It was later confirmed the star had been treated by the onsite medical team, with co-host Massimiliano Rosolino explaining: "Paul is with the medics but the initial view is that he is not seriously hurt."
Reports suggest it's the second injury to hit the show after another contestant


Former Spurs star Paul has been quite the daredevil during his stint on the TV show, which started with him leaping from a helicopter into the sea.
He has also been entertaining the other contestants with tales of his antics during his football hey day, including telling them all about the time he smuggled an ostrich into training.
He's also been sharing some flirty scenes with fellow islander Daniela Martani.


Before joining the show, Gazza is reported to have started taken Italian language classes to help him communicate with the other stars.
Despite spending three years playing for Rome-based club Lazio, his language skills were said to be a bit rusty and he didn't want to appear on screen with subtitles.
During the first episode, Gazza admitted he'd stumbled over his Italian phrases but was hoping to get better as the show went on.
He said: "It's OK. It's only the first day, I'm hoping to improve it."
