NHS North East London Foundation Trust warned over patient suicide

An NHS trust was told to improve the way it accepts mental health patients after a woman took her own life on the day she was sent home from hospital.

Karis Braithwaite, 24, was released less than 24 hours after an earlier suicide bid.

Senior coroner Nadia Persaud wrote to North East London Foundation Trust over deaths where “important risk information” was not passed to mental health staff. An inquest into the death of Ms Braithwaite heard that on September 24, 2018, she stepped in front of a train but was pulled clear and taken to Goodmayes Hospital. Ms Persaud said the way staff accepted a handover from police and paramedics meant the suicide risk was not taken seriously enough.

A trust spokesman said it was “determined to ensure these lessons are learnt”.
