Mix up at the crematorium sees family mourn wrong body during funeral

A family sat through a service for a loved one - not knowing a mix up meant they were mourning the wrong body.

An investigation is under way after the funeral was held at a Coventry crematorium in November.

But as mourners said their final farewells, they did not know the body in the coffin did not belong to their dear departed.

Staff at Canley crem noticed the mistake after the service, and managed to swap it in time before the cremation actually took place.

CoventryLive were told checks supposed to be done before the service didn't happen or didn't happen correctly, meaning nobody spotted the wrong coffin was brought to the service.

Staff at the crematorium, which is owned and managed by Coventry City Council, noticed the mistake following the funeral service.

A spokesman from the council has said a disciplinary process is underway in relation to two members of their staff.

The funeral directors for the service - Henry Ison & Sons, have offered their "sincere apologies" to the family, who have now been made aware of the situation.

A spokesman for the funeral directors said: "We have spoken with the family concerned and given our sincere apologies for the error, which was realised before the cremation took place.

"We are conducting a full investigation to ensure such an error cannot happen again."

Investigation underway

A spokesman for Coventry City Council confirmed an investigation was being carried out.

A spokesman for Coventry City Council said: “We can confirm we are currently carrying out an investigation following a claim a funeral service took place following the delivery of the wrong body by the funeral director to the Chapel at Canley during November this year.



"It is understood that the mistake was discovered immediately following the service and we can confirm that the correct body was cremated.

"However, the family of the deceased was not made aware of this at the time of the service by either the City Council or the funeral director.

"We have now spoken with the family of the deceased and given our heartfelt apologies and the next of kin have asked for their privacy to be respected.

"We are now reviewing our procedures and will make any necessary changes. We have also written to all Coventry funeral directors to ask them to review their processes to further reduce the risk of this happening again.

"We can confirm a disciplinary process is underway in relation to two members of City Council staff.”
