Every day the emergency services go above and beyond the call of duty, often in the most testing of circumstances.
They cannot always get it right – as unfortunately after the Manchester Arena bombing.
While there were individual acts of heroism, rightly recognised, there were also failures.
In the second of his three reports Sir John Saunders found that two of the 22 victims of the terrorist attack might have survived if they had received different care.
His findings will only add to the anguish and grief of the victims’ families.
The police and ambulance services have not sought to hide from the mistakes made and have issued unreserved apologies.
These words are welcome but it is imperative that they act on Sir John’s recommendations so these errors never happen again.
But we should remember that, regardless of the response, the real responsibility for what happened lies with evil bomber Salman Abedi and his despicable accomplices.

The country was already paying the price for the Tories’ mismanagement of the economy for the past 12 years. Now it is paying an additional premium for the idiotic mini-Budget inflicted by Liz Truss and Kwasi Kwarteng.
The Tories should never be forgiven.
