A man who was shot last weekend in Jefferson Park on the Northwest Side has died.

Brian Reyes, 37, died about 12:30 p.m. Tuesday at Illinois Masonic Medical Center, according to the Cook County medical examiner’s office.

Reyes was found lying on the ground with a gunshot wound to the head about 4:15 a.m. March 21 in the 5100 block of West Sunnyside Avenue, Chicago police said.

He was taken to the hospital in critical condition but died two days later, the medical examiner’s office said. An autopsy ruled his death a homicide.

No one is in custody as Area Five detectives investigate.
