The Belgium international, who has been on Spurs' books since 2012, is poised to become a free-agent in the summer, meaning he will be available on a free-transfer.
Spurs, of course, could still tie the defender down to a new deal before his current contract expires in the summer.
However, Milan are sniffing around and could be tempted to make a move for the 32-year-old.
