The Catholic Church has been fined over £13,000 following the death of an 81-year-old parishioner and volunteer.
Christopher Duffy was heavily involved at Sacred Heart Church in Cumbernauld, Lanarkshire.
The 81-year-old was a key holder and responsible for opening the church.
Construction work to install a lift to a hall had been delayed after the builder discovered pipes which he believed were connected to gas and would have to be removed by a specialist contractor.
Fr John Campbell told him they were connected to the water supply and a tank in the loft.
It is believed Mr Duffy went to inspect the pipes, when he plummeted 50ft to his death.
A spokesman for the Archdiocese of Glasgow said: “The Archdiocese wishes to express once more its regret and sorrow.”
