The Eyes Wide With Fear quest can be broken down into two parts.
- Uncover the Survivor's Story
- Find the Madman's Chest
The first task isn't that difficult, as you have a quest marker. The exact location of the chest might vary due to the procedurally-generated nature of the game. You'll have to find the chest where you'll find a clue about what happened to the owner.
Once you get the Diary Fragments from the chest, you can then proceed with the main task, which involves getting the Madman's Chest.
Madman's Chest location in Windrose
The Madman's Chest is located on the same island where you found the remainings of the survivor (the previous stage). The official quest marker will update and highlight a location. The Madman's Chest is similar to the Pirate's Chest, as it's located under the ground.
You'll need to bring out your shovel and use it at the marked location on the image above. The chest is located in an area in-between few big trees. You might need to test a few spots before you get the correct one (you can't go wrong if you refer to the location mentioned in the image above).
You'll find a chest titled "an old chest". Inside, you'll find all the quest rewards you get from completing the task.
