A report from the parliamentary and health services ombudsman has detailed cases where the dying and their families did not get the care they needed.

Julie Mellor’s report includes details about a number of dying patients who went through unnecessary suffering and says too many people spent their last hours in unnecessary pain or distress.

Full details of the report can be read here. We would like to know if the report rings true to your experiences? If so, how do you think end-of-life care services could be improved?
