How better joined-up care could create a more sustainable NHS

With the number of people with more than one long-term condition such as diabetes, asthma or dementia set to rise from 1.9 million in 2008 to 2.9 million in 2018 and increasing pressures on A&E departments, the need to deliver better joined-up care and a more sustainable NHS has never been more urgent.

Diagnosis, care planning, patient safety and the efficient use of ever decreasing clinical resources, are intrinsically linked to the efficient and secure exchange of information between care providers, throughout the patient care pathway journey. To facilitate a true patient-centric view throughout the patient journey, medical records from multiple disparate health information systems must be made available to each of the often autonomous co-operating care provider organisations, while maintaining patient confidentiality and complying with information governance policies.

The latest information can then be presented through the intuitive user interface designed by Fortrus, which is engaging for users and has been created with the user experience as its key focus.

This in turn enables greater, more complex information to be accessed more readily and accurately than through any other means. The key is that the solution offered by Fortrus and Tiani-Spirit ensures that better joined-up care can actually be delivered, which is made possible because it has been designed by clinicians, for clinicians and is therefore based on the actual user requirements ensuring ease of access to data from multiple systems, which is then presented to the users in a single intuitive viewer.

This unified view of critical information developed by Fortrus also gives users multiple ways in which to access the data, for example; the Fortrus Timeline creates a dynamic chronological view of all information relating to a patient and their complete health records.

By working together Fortrus and Tiani-Spirit provide a comprehensive solution that brings together and presents information in a way that is easy for clinicians to digest, enabling them to make accurate and efficient diagnoses. The intuitive aspect of the system offsets the need for comprehensive training, which coupled with the comparatively short implementation time, enables organisations to rapidly see a return on their investment whilst creating a more sustainable NHS in a relatively short space of time.
