The right-back signalled to the bench to come off just after the hour mark.
Smith, 33, hobbled off with clear discomfort against Switzerland.
And he could now be touch-and-go for the first Edinburgh clash of the campaign on Sunday afternoon.
The veteran defender did not have his best game in Northern Ireland colours, giving away a penalty in the first-half.
But he was bailed out and his side held onto a goalless draw despite hauling down Ruben Vargas inside the area.
Robbie Neilson will be sweating on Smith's fitness this weekend as Tynecastle prepares to host the opening derby of the campaign.
Cammy Logan, meanwhile, remains sidelined so there appears to be no clear replacement for the full-back.
Both Edinburgh outfits sit joint top of the Premiership table.
Neither side have lost a match this season so far.
