A toddler is reported to have tragically slipped from her grandfather's grasp and fallen from the 11th floor of a cruise ship.

She was rushed to hospital but pronounced dead shortly afterwards.

At the time of the incident the ship was understood to be berthed at the Pan American dock II in San Juan, Mirror Online reports.

The girl and her family, including her maternal and paternal grandparents, parents and a younger brother, are from Indiana, local outlet Primera Hora reported.

The girl was from Indiana, USA, and investigators are interviewing passengers who witnessed the fall and the girl's relatives to ascertain exactly what happened.

Freedom of the Seas is the 15th largest passenger ship in the world and accommodates just shy of 5,000 passengers and crew.

The young girl is tragically not the first to fall from the upper decks of a cruise ship.

In 2012 a 14-month toddler fell from deck 12 to deck 11 on the Monarch of the Seas.

The ship turned back to Port Canaveral in Florida where the child wad given urgent medical attention and recovered.

Royal Caribbean has been contacted for comment.
