The Belgian midfielder, whose time at Chelsea has been disrupted by several fitness setbacks, pulled up after holding off Leandro Andrade in a defensive duel near the edge of his own box.
Lavia was able to complete the clearance but limped away before falling to the ground to receive treatment.
Enzo Maresca opted to take no chances with the 21-year-old and immediately substituted the midfielder and should provide an update in the post-match press conference.
Chelsea made the trip to Azerbaijan without several first-team players including Cole Palmer and Benoit Badiashile, while Pedro Neto became the latest casualty after missing training on Tuesday.
Though the Portuguese winger is not expected to be sidelined for too long.
“[It’s] not a problem, just a small issue,” Maresca told reporters. “We will try to protect him. He is not involved in tomorrow's game.”
Following the Qarabag clash, Chelsea return to Premier League action at the weekend as they welcome a Wolves side without a manager to Stamford Bridge as they seek to go into break on a positive note.
