Leeds United are heading back to the Premier League having been confirmed as champions, and will be joined by West Brom.
Brentford, Fulham, Cardiff City and Swansea City are into the play-offs, while Charlton Athletic have been relegated along with Hull City - and Wigan Athletic will join them unless the Latics' appeal against a points deduction is successful.
