They join Norwich City and Sheffield United in winning promotion while at the other end of the table Ipswich Town, Bolton Wanderers and Rotherham United have suffered relegation to League One.
Huddersfield Town, Fulham and Cardiff City have been relegated from the top flight and join the Championship next season, while Charlton Athletic, Luton Town and Barnsley have earned promotion from the third-tier.
