BRIEF: Hoffman Estates man dies after vehicle crashes with ambulance in Streamwood

Nov. 23--A Hoffman Estates man died Sunday after an SUV he was driving crashed with an ambulance that was transporting a patient in Streamwood, officials said.

Four other people were injured in the crash, which happened about 4 p.m. near the intersection of Illinois Route 59 and Tallgrass Court.

Jitendra Shah, 64, was pronounced dead at 5:30 p.m., according to the Cook County medical examiner's office.

According to a news release from the Streamwood Police Department, Shah was driving a 2005 Honda CR-V south on Route 59 when he crossed into the northbound lanes and struck a private ambulance operated by Trace Ambulance Co.

Shah, a 53-year-old passenger in his vehicle, and the three occupants of the ambulance were taken to St. Alexius Medical Center in Hoffman Estates, where Shah died, according to the news release. His passenger was in serious condition, and three occupants of the ambulance suffered non-life-threatening injuries, according to the release.

The incident remains under investigation. Weather and road conditions may have been a factor in the crash, police said.
