One woman has been killed in a huge pile-up involving up to 20 vehicles on the A40 in Oxfordshire.
Up to 15 people are being treated for minor injuries while another is thought to be in a serious condition.
A stretch of the major road near the town of Witney has been closed while authorities attempt to help the injured and clear the debris.
Images on social media showed badly-damaged vehicles strewn across the road following the crash, which took place at 8.22 on Wednesday morning. A van can be seen upside down in a ditch.
A spokesman for the South Central Ambulance Service said several people had been seriously injured. Three ambulances, three rapid response vehicles and an air ambulance car are at the scene.
Witnesses said the accident happened in thick fog and "very icy" road conditions.
