A Shrimp Virus Was Linked to a Blinding Human Eye Disease, and Then the Journal Flagged the Paper

In March, a team in China reported something the authors said had never been documented: a virus that lives in shrimp and fish appeared to be infecting human eyes and causing a severe, treatment-resistant disease. In July, the journal that published it told readers to be careful.

The paper, published in Nature Microbiology on March 26, linked covert mortality nodavirus, or CMNV, to persistent ocular hypertensive viral anterior uveitis, abbreviated POH-VAU. It drew heavy international coverage.

On July 7, Nature Microbiology attached an editor's note stating that concerns have been raised regarding the reliability of data presented in the article, and that further editorial action will be taken if appropriate once the investigation is complete and all parties have had an opportunity to respond in full.

That note belongs in any current account of this story.

What the Paper Reported

The evidence chain was unusually layered, which is part of why the finding landed hard.

POH-VAU presents as recurrent inflammation in the front of the eye with extreme pressure spikes. Among the 70 enrolled patients, 82.9% had more than one episode and 51.4% had peak intraocular pressure above 45 mm Hg, more than double the 21 mm Hg upper limit of normal. Repeated episodes damage the optic nerve, strip the corneal endothelium, and atrophy the iris. Roughly a third of patients needed glaucoma surgery despite medication. Patients tested negative for the usual culprits, or carried only trace levels: herpes simplex, varicella zoster, cytomegalovirus, Epstein-Barr.

In iris tissue removed during surgery, electron microscopy showed viral particles around 25 nanometres across. Gold-labeled anti-CMNV antibodies bound them. Mass spectrometry pulled peptide fragments of the CMNV RNA-dependent RNA polymerase out of patient tissue. Sequenced gene fragments matched CMNV from aquatic animals at 98.96%. All 70 patients showed antibody seroconversion, while 27 healthy controls did not.

Mice injected with the virus into the vitreous showed intraocular pressure 26.2% above controls by day 28, alongside corneal, iris, and retinal damage. Human corneal epithelial cells died in culture.

Hands, Not Mouths, Carried the Bigger Signal

The exposure survey produced the finding most likely to matter practically, if it holds.

Unprotected processing of aquatic animals was the suspected highest-risk exposure mode at 54.3% of cases, ahead of eating raw aquatic products at 17.1%. Together, those accounted for 71.4% of investigated patients. Home-based handlers made up 51.4% of patients, while full-time commercial processors, who use protective equipment, did not emerge as a high-exposure group.

Puncture wounds from fins, shells and appendages of live or frozen animals ranked as the single highest-risk event. Logistic regression adjusted for age and sex produced odds ratios of 6.14 for the number of severe exposures and 10.91 for exposure severity, though after stepwise regression only the number of severe exposures survived as an independent risk factor, at 3.35. Median incubation from suspected exposure to onset was six months.

CMNV itself is genuinely widespread. A survey of 523 samples found it across Asia, the Americas, Europe, Africa and Antarctica, in 49 species, with the highest positivity in Antarctic krill. The World Organization for Animal Health listed CMNV infection as an emerging disease in 2023, and maintains a technical disease card describing it as a shrimp aquaculture pathogen.

What an Editor's Note Means

An editor's note is not a retraction. It is a formal signal that a journal has received concerns it considers substantive enough to investigate and to flag for readers while it does.

The paper had already been amended once, with an author correction published May 5. Nature Microbiology has not specified publicly which data are in question. The article remains available and continues to be cited.

Until the investigation resolves, the honest position is that the central claim is unverified rather than either confirmed or debunked.

The Uncertainty That Predates the Flag

The authors themselves listed limits that constrain the finding regardless of how the review ends.

They describe CMNV as a possible causative agent, not a proven one, and note that case-control designs have inherent limits for inferring causation and that their sample sizes were insufficient for a solid exposure risk assessment. Recall bias affected the exposure and incubation data, which came from just 36 patients. The provincial aquaculture correlation was an ecological analysis, non-significant for 2019 to 2021 and significant for 2022 to 2024, which the authors say cannot be generalized to individuals.

One number complicates the picture in both directions: 38.7% of patients without POH-VAU but with pressure above 20 mm Hg also tested CMNV-positive, which the authors read as possible latent infection. The mouse work used direct injection into the eye, not ingestion or a hand wound, so it shows the virus can damage ocular tissue, not that the proposed route of human exposure produces the disease. The authors raise the possibility of interfamilial transmission based on a handful of urban cases, but say it requires further validation.

No cases have been described outside China, and there is no evidence of an epidemic.

Wearing gloves when handling raw fish and shellfish, and avoiding cuts from shells and fins, is sound kitchen hygiene for reasons unrelated to this paper. Anyone with a red, painful eye, light sensitivity, or blurred vision should see an eye doctor promptly, whatever the cause.
